Building Services for Booking with New Multi-scale and Simple Questionnaires

Create and manage services for booking with the new format of questionnaires that can be used across Cosmetic & Health Procedures.

1. Head to Appointments through the web.

2. Locate the Wrench Icon to the right of the calendar, and click Smart Configuration.

3. Click Services.

4. You can either click Create Service to create a new service, or you can click into existing services and edit if you do not need to create a new one.

5. Be sure to fill out all of the details at the top for Service Name, Provider, Duration, etc., then scroll down to Select Questionnaires.

6. In this list you will see both Simple and Multiscale Questionnaires, as these are now universal between both the Cosmetic and Health Procedures and can be used through either procedure type.

With this, you no longer need to select if you are creating a Cosmetic or Health service here.

  • Simple Questionnaires are indicated by a standard paper icon next to them.

  • Multiscale Questionnaires are indicated by a paper icon with a scale symbol on it.

7. After selecting a Questionnaire from the list, you can click the dropdown again to add any additional forms to the service.

Assigning questionnaires and consents through this section of your account, will automatically send attached paperwork to the Patient's Portal for completion each time this specific service/ appointment is scheduled.

When creating Cosmetic or Health Procedures through the Provider App, you will now have the ability to bypass paperwork by Mark on File.

8. Example here, the Treatment History Form is a Multiscale Questionnaire, and the Skin Check is a Simple Questionnaire.

9. Make sure you have necessary Consents, Resources, Treatment Instructions, and all other required and important fields filled out. Then you can click Save to update.
