A consent form allows you to acquire patient consent before proceeding with a treatment.
A. Add a Consent
Log in to your AR account via https://app.aestheticrecord.com/login to create a consent and click on Settings, Clinical Documentation, Patient Paperwork, Consents, and then Create Consent.
Type in or copy and paste the consent title and the consent description. In addition, you can choose to require the patient to sign on each page and/or require a witness signature, and then hit Save.
To attach a consent to a service so that when the service is booked by a patient the consent form will show up in the patient's portal account, please click here.
B. Edit a Consent
Log in to your AR account via https://app.aestheticrecord.com/login to edit a consent and click on Settings, Clinical Documentation, Patient Paperwork, and then Consents. Click on Edit, make the changes, and then Save.
C. Delete a Consent
Log in to your account via https://beta.aestheticrecord.com/ to delete a consent and click on Settings, Clinical Documentation, Patient Paperwork, and then Consents. Select the consent form you want to delete, click on Edit, and then Delete.
