Medical History Form
The Medical History Form can be accessed in the Patient Portal (Patient Facing) and from the Aesthetic Record App (Clinic Facing). This information lives in the clients profile and can be updated at any time. You can also use the @call function while note taking to bring any information from the form to your notes.
To fill out the Medical Questionnaire via iPad/iPhone:
- Launch the application.
- Log in to AR.
- From the main menu, select "Patient List".
- Select patient.
- Click the three dots in the top right corner
- Press Medical History
- Go through the questionnaire with your patient.
- Press save in the top right corner.
- This information can then be pulled with the @call function during note taking
If the form has the Drug Allergy Section filled in, it will badge the client profile with a red caution sign to alert you of any potential risks.
