Manage Cosmetic Questionnaires

Add, edit, or delete a questionnaire; attach it to a service via web

A cosmetic questionnaire is a set of questions that you set up for your patient to answer at the time of treatment or from the patient portal prior to their visit.

A. Create a Cosmetic Questionnaire

Log in to your account via https://app.aestheticrecord.com/login to add a cosmetic questionnaire and click on Settings, Clinical Documentation, Patient Paperwork, Cosmetic Questionnaires, and then Create Questionnaire.

The questionnaire name, question(s), and question type need to be added on this page.

Yes/No: For instance, you can ask the patient to specify the medications that they're taking by asking "Are you taking any medication? If yes, please specify below." If they select Yes, a textbox below the question will show up where they can type in their answer. If they select No, they will simply be prompted to move on to the next question.

If you don't want a textbox associated with your Yes/No question, we recommend that you use the Multiple Selection - Text option instead and put the Yes/No as the only available answers there.

Multiple Selection - Text: This option will add a multiple choice question. Click +Add under Question Choices to add another answer. Be sure to add a "None" option as an answer choice.

Multiple Selection - Image: This option is similar to Multiple Selection - Text. The only difference is you'll use an image for answer instead of a text. Click on Choose File to upload an image to be used as your answer. If you want to, you can also display labels for your image.

Click Add once you've finished filling out the question. Continue to add more questions this way. Hit Save once you've added all the questions for your questionnaire.

B. Edit a Cosmetic Questionnaire

Edit a questionnaire by logging in to your account via https://app.aestheticrecord.com/login and clicking on Settings, Clinical Documentation, Patient Paperwork, Cosmetic Questionnaires, select the questionnaire you want to edit, and then Edit. After editing the questionnaire, hit Save.

C. Delete a Cosmetic Questionnaire

You can delete a questionnaire by logging in to your account via https://app.aestheticrecord.com/login and clicking on Settings, Clinical Documentation, Patient Paperwork, Cosmetic Questionnaires, select the questionnaire you want to delete, and then hit Edit. Scroll down to the bottom of the Questionnaire and hit Delete in the bottom left corner.

D. Attach a Cosmetic Questionnaire to a Service

Attach a questionnaire to a service by logging in to your account via https://app.aestheticrecord.com/login and clicking on Settings, Clinical Documentation, Patient Paperwork, Cosmetic Questionnaires, select the questionnaire you want to attach, and then Edit. At the top, there will be an Assign to Services button. Click it and choose the services/services you wish to attach the questionnaire to.

You can also attach a questionnaire by going to Appointments, Wrench Icon, Smart Configuration, Services, and then click directly on the service you wish to attach the questionnaire to. Scroll down to the bottom where it says "What type of questionnaire you want to associate? and click on Cosmetic. From the dropdown menu, select your questionnaire. You can select multiple or just one.

Once the questionnaire has been assigned to a service, it will upload to the patient portal at the time their appointment is booked for them to fill out ahead of time.
