Manage Health Timeline Questionnaires
Follow this description to create your own templates via web.

A health timeline questionnaire gives you the ability to create and customize a form that is used to track specific procedure details. Keep in mind that these questionnaires are used for health procedures.

A. Create a Health Timeline Questionnaire

Log in to your account via https://app.aestheticrecord.com/login to add a health timeline questionnaire and click on Settings > Clinical Documentation > Patient Paperwork > Health Timeline Questionnaires > Create Questionnaire.
Important note: This will only show up if you create a health procedure.

Give your health timeline questionnaire a name (for example, "Health Screening").

Under the title, you can add in your first question (for instance, "Are you over 21?").

If you want to duplicate this same question, you can click on the two overlapping squares icon next to the trash bin icon. If you want to delete this question, simply hit the trash bin icon.

You can turn on the Required toggle if you don't want the patient to skip this specific question. You can turn on the Description toggle if you want to add a short description of this question.
On the right, you can see how the question is seen from the patient's perspective.

If you want to add another question, simply hit Add Question in the top right and then hit Save next to it to save the entire questionnaire.

Where it shows Yes/No on the left, that's where you can select other question types:

Textbox if the patient/provider has to type in their answer.

Yes/No if the patient/provider needs to choose between yes or no.

Single Choice if the patient/provider can only choose one answer from various options.

Multiple Choice if the patient/provider can choose more than one option.

Opinion Scale if the patient/provider needs to select a number (from 1-10).

FIle Upload if the patient/provider needs to upload a file as their answer.

B. Edit a Health Timeline Questionnaire

Log in to your account via https://app.aestheticrecord.com/login to edit a health timeline questionnaire and click on Settings > Clinical Documentation > Patient Paperwork > Health Timeline Questionnaires > Look for the one you want to edit > Edit.

C. Delete a Health Timeline Questionnaire

Log in to your account via https://app.aestheticrecord.com/login to edit a health timeline questionnaire and click on Settings > Clinical Documentation > Patient Paperwork > Health Timeline Questionnaires > Look for the one you want to edit > Edit > Delete in the top right.


D. Attach a Health Timeline 

Questionnaire to a Service
Attach a questionnaire to a service by logging in to your account via https://app.aestheticrecord.com/login and clicking on Settings, Clinical Documentation, Patient Paperwork, Health Timeline Questionnaires, select the questionnaire you want to attach, and then Edit. At the top, there will be an Assign to Services button. Click it and choose the services/services you wish to attach the questionnaire to.

You can also attach a questionnaire by going to Appointments, Wrench Icon, Smart Configuration, Services, and then click directly on the service you wish to attach the questionnaire to. Scroll down to the bottom where it says "What type of questionnaire you want to associate? and click on Health Timeline. From the dropdown menu, select your questionnaire.  You can select multiple or just one. Click Save.

Once the questionnaire has been assigned to a service, it will upload to the patient portal at the time their appointment is booked for them to fill out ahead of time.
If the provider wants to fill out the questionnaire instead, simply do not assign to a service and fill out the questionnaire in office during appointment.
