Patients can register for the patient portal so they can complete the forms prior to their appointment date via the web
If patients are successfully registered for the patient portal, they can read the pre and post-treatment instructions, answer questionnaires, and sign the consents tied to the service(s) they booked prior to their visit via a web browser on any device.

A. Register for the Patient Portal (as the Patient)
When the clinic creates a brand new patient profile for you, you'll automatically get an invite via email to register for the portal:
The patient has two options:
- They can go to your patient portal URL and hit "register for the patient portal"
- They can hit "confirm my account" in the welcome email that goes out to them
Once they are at your URL for the patient portal, here is what they will see:
From this point, they can enter existing details OR register. Please note: the patient MUST use the same email address that is on file to tie their patient portal to their AR profile.
Next, if they are registering for the first time, they need to enter in their details.
Once they hit register, they need to go to their email address and verify.
Once they hit "activate your account", it will take them back to the patient portal login screen. Where they can enter their credentials and hit login.
Once the patient profile is active they can login to complete forms.
Log in to your patient portal account. On the left side, click on the pre-treatment instructions that you need to read, questionnaires that you have to complete, and consents that you're required to sign prior to your visit.
Once you're done, your provider can view those documents by clicking More Info and then Documents on your profile via a browser.
