Creating & Using Provider Questionnaires

Provider Questionnaires allow your clinical team to create question-based charting templates that are not sent to the patient’s portal. These questionnaires are designed purely for internal medical documentation during Cosmetic or Health procedures.

How to Use Provider Questionnaires (Internal-Only Charting Forms)

What Are Provider Questionnaires?
Provider Questionnaires are internal charting templates that force navigate the provider through a series of questions so they can provide complete documentation of the procedure.

  • Do not send to the Patient Portal

  • Can be attached to services

  • Appear as required documentation during the procedure

  • Are stored in the patient’s chart & document repository

  • Can be completed on Web and iOS

This gives providers the flexibility to complete questionnaires without exposing internal forms to patients.

1. Creating Provider Questionnaires
Where to find them
Go to:
Settings → Clinical Documentation → Client Paperwork → Provider Questionnaires
Here you can:

  • Create new Provider Questionnaires

  • Edit existing ones

  • Reorganize or delete forms

  • Copy existing questionnaires over into a Provider Questionnaire

These behave exactly like multiscale questionnaires but are internal only.

2. Moving Existing Multiscale Questionnaires Between Buckets
We have provided an easy button to facilitate moving your questionnaires if you already have a similar version created for patients or vice versa.

Move options available:

  • Move to Provider Multiscale Questionnaires

  • Move to Patient Questionnaires

This prevents users from needing to recreate forms from scratch.

3. Assign Provider Questionnaires to Services
Provider forms can be automatically required in procedures.
To assign:
Go to Settings → Smart Configuration → Services → Edit Service
You’ll now see a Provider Questionnaires dropdown.
Select one or more templates.
These forms will automatically appear when performing:

  • Cosmetic Procedures

  • Health Procedures

  • (On Web & iOS)

4. Identifying Provider Questionnaires in Procedures

  • Provider forms appear with a red eye icon to indicate they do not go to the patient.

5. Completing Provider Questionnaires During a Procedure
When a provider begins a procedure:

  • Any assigned PQs are shown as required

  • Providers complete them directly within the chart

  • You can mark them to "Finish Later" if you have a procedure that needs multiple charting time points (i.e. an IV where you need to take vitals at various points in the procedure)

  • They are stored in the patient’s Document Repository

  • They are never sent to the Patient Portal

You can also manually add additional PQs while charting.

6. Viewing Completed Provider Questionnaires
Provider Questionnaires can be viewed in:

  • The patient’s Document Repository

  • The completed Procedure details screen

The Provider/MD Room
