1. First, head to Settings through the web.
2. Click on Clinical Documentation on the left, and then Patient Paperwork.
3. Let's look at Simple Questionnaires. Click Create Questionnaire over to the right.
4. Be sure to enter a title for this questionnaire in the "Questionnaire Name" field.
5. Next, click Add new question to start building out the questions for this form.
Creating a Yes/ No Question Type
6. There are three question types you can choose from when building a Simple Questionnaire.
Yes/No will prompt the patient to select either yes or no, and if the patient selects yes when filling the questionnaire out, a text box will pop up and prompt them to provide any relevant information for their answer.
We'll look at the other two question types in the steps below.
Creating a Multi Text Question Type
7. The Second Question Type, Multi Text, is a multiple choice question type. Here, you can list out multiple choices for the patient to select just one, or multiple if applicable.
8. If you check the box for Multi Selection at the top, this will allow multiple answers to be selected. Leave this unchecked if you need the patient to select only one of the choices below.
9. Should you need to delete an answer choice, you can do so by clicking the Red trash can icon to the right of the question to remove.
After creating this question, be sure you click Add at the bottom to save!
Creating a Multi Image Question Type
10. The Third Question Type, Multi Image, allows you to upload multiple choices as images for the patient to choose from. An example here would be uploading images of different skin types for the patient to see and select a visual representation.
11. The Multi Selection checkbox at the top will allow the patient to select more than one image/visual within this question.
12. The Display Labels checkbox is a helpful tool to use if you want to label the images for more clarification.
13. Remember! As you create your questions, be sure to click Add after each question to save your questions before proceeding.
Manage Existing Questions
14. As you continue to create and save your questions, you will see each question add to the list.
If you need to edit or delete an existing question during this time, you can click the pencil or trash can icon to the right of the question. This will either take you back into the specific question where you can make any edits, or delete the question for you.
If you need to adjust the order of the questions, locate the two lines to the left of a question. From here, click and hold, then drag the question up or down on the list.
Assigning the Questionnaire to Services for Booking
15. When you have finished building out your questions, the last step is to assign your questionnaire to any relevant services for booking.
Keep in mind, when you assign a questionnaire to services for booking, this questionnaire will automatically send to the patient's portal upon their appointment being scheduled. This is a highly recommended flow if you want to encourage patients to complete paperwork prior to their visit.
Locate the Assign to Services button in the upper right hand corner.
16. This will give you a list of ALL services for booking you have created under your Smart Configuration tab. (If you do not see a service name here, you will need to go back to Appointments > Smart Configuration > Services and create the service for booking there.)
You will first see each Service Category. If the questionnaire needs to be assigned to every service within the category, check the box to the left.
If you only need this questionnaire assigned to a specific set of services within the category, use the small arrow to the right of the category name to see and select specific services from there.
Helpful tips for navigating the services list:
You can utilize the search bar on the left for a specific service name.
Utilize the dropdown in the right hand corner to filter between the list services that the questionnaire is/isn't already assigned to.
17. After assigning the questionnaire to all needed services for booking, be sure you click Assign at the bottom to save!
18. Finally, double check once more that you have
Your Questionnaire named
Questions created
Assigned to services
Then save!
