Create and Manage Multiscale Questionnaires for Patient Appointments

1. From the web, click Settings in the right corner.

2. Go to Clinical Documentation > Patient Paperwork.

3. Make sure you are under the Multiscale Questionnaires column, and click Create Questionnaire.

4. First, give your questionnaire a name. Next, let's click Add new questionnaire below.

5. Multiscale Questionnaires have more question types and features to choose from than Simple Questionnaires, as they are intended for longer, and more intricate information to collect.

We will walk through each question type in the steps following.

Creating a Text Box Question Type

6. The Text Box question type for capturing open-ended information from the patient as they fill it out. On the patient's end, they will read the question, and see a text box below to provide their answer.

  1. Add the question in the Question section.

  2. If this question is required to be filled out, check the box next to Required.

  3. The Description section is for you to emphasize on any additional information that the patient can use for reference, as they provide their responses. This section is optional

7. If you need to erase and start over on a question, you can click Reset- this will clear any information you have entered so you can begin from scratch.

Before exiting a question, be sure you hit Add, to save your progress.

8. After the creation of each new question, you will be brought back to the overall questionnaire where you can continue adding new question types.

Creating a Yes/No Question Type

9. The Yes/No question type will prompt the patient to select either yes or no as they complete the answer on their end.

Again, you can choose to make the question required and/or provide a description if needed.

**After reviewing all question types, we will look at utilizing the Logic Jump Feature!**

Creating a Single Choice Question Type

10. The Single Choice Question Type is for creating multiple answer choices, but limiting the patient to only choosing ONE.

This option is good to use for something like skin type, or a question where there may be multiple choices to choose from, with only one answer being applicable.

Below are features within this question type that you can utilize:

  • Marking the question as Required to complete.

  • Providing a Description for reference to the question.

  • Deleting an answer choice by using the red trash can icon to the right.

  • Reordering the answer choices by using the two lines to the left of an answer, clicking and holding, then dragging the answer up or down.

Creating a Multiple Choice Question Type

11. The Multiple Choice Question Type is for creating multiple answer choices, and allowing the patient to select more than one if applicable, and without limiting them to a single answer.

This option is good to use for something like asking the patient to select all treatments they are interested in learning more about or receiving at your practice.

Below are features within this question type that you can utilize:

  • Marking the question as Required to complete.

  • Providing a Description for reference to the question.

  • Deleting an answer choice by using the red trash can icon to the right.

  • Reordering the answer choices by using the two lines to the left of an answer, clicking and holding, then dragging the answer up or down.

Creating an Opinion Scale Question Type

12. Opinion Scale is going to be a rating/likelihood scale with a standard 1-10 set of numbers.

  • When completing this question type, patients will see numbers 1-10 and be able to select the relevant number based on the question.

  • With Opinion Scale questions, if you need the patient to also provide any details for their rating choice, you can check the box for Comments on Scale.

  • This will prompt a text box below their selected number, asking them to provide a further explanation for their choice.

Creating a File Upload Question Type

13. The last question type is File Upload. This option is going to prompt the patient to upload a photo or file of what you are asking for, when they fill out the questionnaire.

A good use case scenario for this question type could be asking them to upload a photo of their Driver's License, or specific document related to their patient information.

One thing to keep in mind here*, is if a patient uploads a file or photo through this section, it will only live within the questionnaire and will not become a file inside of their Document Repository. You will need to manually screenshot the file and upload separately if you need it housed elsewhere.*

Utilizing the Logic Jump Feature

14. Now that we have looked at each question type, let's walk through utilizing the Logic Jump feature for your questions!

  • The Logic Jump feature is extremely beneficial to use for a long questionnaire in which not all answers are going to be applicable.

  • For example, if a patient answering a question about their last treatment is only dependent on if they chose yes to receiving a treatment previously, logic jump would allow the patient to skip the question about their last visit, if they answer no instead.

  • Helpful Tip! We highly recommend building out each question type and adjusting the order of any questions before incorporating the Logic Jump, so that you have all questions and correct formatting to pull from as you map out these steps.

To incorporate Logic Jump for a question, first click the pencil icon as if to edit the question.

15. Then, click Logic Jump at the bottom.

Please Note: Text Box, Multiple Choice, File Upload do not contain the Logic Jump feature at this time.

16. From here, you will be able to utilize the sections and direct the flow of the questionnaire based on what the patient chooses.

  • If Selected refers to the current question and answer choices.

  • Then jump to is where you will direct the flow to the next question based on what you put in the If Selected above.

  • Else jump to will prompt you to choose a different question to direct the flow to, for all other answers given than in the If Selected section above.

If you need to add an additional Logic Jump rule within the same question, you can do so by clicking + Add Logic Jump and continue the same steps.

17. If you need to reorder the questions within a questionnaire, locate the two lines to the left of the question, and click, hold + drag the question up or down.

18. When managing existing questions within a Multiscale Questionnaire:

  • Utilize the Pencil Icon to access and edit a question.

  • The blue Duplicate Icon will make a copy the original question for you to click into and edit where needed.

  • The Red Trash Can Icon will delete a question if you no longer need it.

Reminder, always Save your work before moving on!

Assigning your Questionnaire to Services for Booking

19. After creating and saving a Multiscale Questionnaire, you will want to Assign it to Services for Booking.

To do this, click Edit into one of the questionnaires from the main screen.

20. In the upper right hand corner, click Assign to Services.

Keep in mind, when you assign a questionnaire to services for booking, this questionnaire will automatically send to the patient's portal upon their appointment being scheduled. This is a highly recommended flow if you want to encourage patients to complete paperwork prior to their visit.

21. This will give you a list of ALL services for booking you have created under your Smart Configuration tab. (If you do not see a service name here, you will need to go back to Appointments > Smart Configuration > Services and create the service for booking there.)

  • You will first see each Service Category. If the questionnaire needs to be assigned to every service within the category, check the box to the left.

  • If you only need this questionnaire assigned to a specific set of services within the category, use the small arrow to the right of the category name to see and select specific services from there.

Helpful tips for navigating the services list:

  1. You can utilize the search bar on the left for a specific service name.

  2. Utilize the dropdown in the right hand corner to filter between the list services that the questionnaire is/isn't already assigned to.

Deleting / Restoring Questionnaires from your Account

22. If a Multiscale Questionnaire within your account is no longer needed, you can click Edit into the questionnaire, and Delete at the bottom.

This will not delete any questionnaires that were previously completed within a patient's Document Repository. However, this action will remove the questionnaire from any services for booking it was assigned to, as well as remove it from being utilized moving forward.

23. You will see a confirmation pop-up to ensure you want to move forward with deleting this Questionnaire from your account.

24. If you accidentally delete a questionnaire, you can restore it to your account by going to Recently Deleted under Settings > Paperwork.

25. From here, select the relevant column for the type of paperwork you are trying to restore from.

Depending on when the Questionnaire/paperwork type was initially created, you can use the date filter at the top to search for the title of paperwork you are looking for.

26. Use the check boxes to the left of the paperwork in which you are needing to restore, or you can select multiple at once by using the check box at the top left next to the Title column.

Click Restore in the upper right hand corner when you are ready to move on.

27. Lastly, you will need to confirm the restoration of paperwork to your account, which should take place within about 10 minutes of the action being finalized.
