Manage Consent Forms

A consent form allows you to acquire a patient consent before a treatment. Consents can be signed in office or from the patient portal.

1. Log in to your AR account via the web.

2. Click the Settings Gear in the top right corner.

3. Click "Clinical Documentation"

4. Click "Patient Paperwork"

5. Click "Consents"

6. Click "Create Consent"

7. Type in or copy and paste the consent title and the consent description.

8. In addition, you can choose to require the patient to sign on each page and/or require a witness signature.

  • Initials on pages: When this is checked for a consent, in addition to the patient providing their signature at the end of the consent, they will also need to account for their initials on each page. This is an additional protection against you and your patients, and is a required action if enabled.

  • Witness Signature: When this is checked for a consent, in addition to the patient signing the consent after reading through, a witness will also be required to provide their signature at the time of the visit in order for the consent to be marked as fully complete through the chart.

9. AR users can upload their consent forms by clicking on the "Upload Files" option at the top.

10. You can also edit an existing consent form by clicking Edit.

11. Additionally, after creating consent forms, users can attach them to services by clicking on "Assign to Services" located at the top.
