Manage Cosmetic and Health Questionnaires for Patient encounters in Aesthetic Record

Manage Questionnaires in Aesthetic Record

2. Head to your settings wheel in the upper right hand corner.

3. On the left, navigate to and click "Clinical Documentation"

4. Select "Patient Paperwork".

5. Let's start with the Cosmetic Questionnaire tab first, and click "Create Questionnaire"

6. Give your questionnaire a name! (Injectables First Time for example)

7. You can type in your questions that you need added to the form.

8. Yes/No questions will allow patients to choose a checkmark or an X when filling the questionnaire out. If patients choose the checkmark, this will prompt a text box for them to further specify any relevant information when filling it out.

9. Click "Add" to save this question and start on the next one.

10. Click the dropdown for additional question types:

Multiple Selection-Text: this allows you to list out multiple answer options for the patient to choose one. Be sure to add in a "none/other" option too, so they can select that choice if nothing applies.

11. Begin building this type of question and question choices now.

12. Click "+ Add" to add each new answer choice.

13. Click "Add" at the bottom to save the overall question and question choices when you are finished.

14. After finishing your questionnaire, be sure to click "Save"!

15. You can also edit questionnaires from your main questionnaire list by clicking the "Edit" button to the right.

16. You can Edit or Delete individual questions within the overall form as needed.

17. "Assign to Services" is a quick and easy way to assign questionnaires to their respective services for booking.

Keep in mind, assigning questionnaires to services enables the questionnaire to automatically send to the patient's portal as a required document, upon their appointment being scheduled!

18. Select from overall Categories or individual Services to assign this questionnaire to. Be sure to click "Assign" when you are finished before leaving the screen.

19. Now let's look at creating "Health Timeline Questionnaires"

Health Questionnaires are specific to procedures created through a Health Procedure/Encounter, so you want to make sure that you build your services for booking according to which Questionnaire type you build through this section.

20. Switch over to Health Timeline Questionnaires and click "Create Questionnaire"

21. These questionnaires are more multi-scale, and are built differently than Cosmetic Questionnaires.

In the "Title" Box, give this questionnaire a name.

22. Begin typing in your first question.

23. You can use the dropdown to the left to choose different question types:

Textbox: open ended answers

Yes/no: enables a logic jump feature to pass over questions that don't fit certain criteria

Single choice: allows for one answer to be selected

Multiple choice: allows for multiple answers to be selected

Opinion scale: 1-5 grade scale for selection

File upload: can upload answer choices as pictures

24. You can also make a question required or optional, and you can add a question description if needed.

25. After each question you make, click "Add Question" in the upper right hand corner to continue to the next question.

26. When you have finished creating your questionnaire, be sure to hit save!

27. As with Cosmetic Questionnaires, you can click "edit" into already created questionnaires and assign them to services for booking.

28. Select from the overall service categories, or use the dropdown arrows to choose individual services.

29. Click "Assign" after selecting the categories/services for this questionnaire before exiting.
