Create a ChartSmart AI Encounter Note on the Web

Follow these steps to utilize ChartSmart, your AI-powered charting co-pilot for a the Health Procedure or Cosmetic Note via Web Browser

Please Note: You will need a ChartSmart subscription to utilize this feature. Choose from our plants by visiting Settings > Billing > Subscriptions > ChartSmart, and make your selection to get started.

Creating a Health Procedure with ChartSmart

1. Start by clicking Create Procedure

2. Click Health

3. Complete your chart information, and click Start Dictation. You can either dictate a note or you can use it as an ambient charting tool by starting your dictation at the start of the procedure.

You will have to allow access to your browser to access your microphone. This is required for the tool to record the audio.

4. While the dictation is recording, you will not see the notes transcribe. As long as it is red, you are recording!

5. Once you click Stop Dictation, the system may take a few seconds to transcribe the audio, but once complete, you will see the transcript appear in the procedure note section.

6. If you would like to add any information to the original transcription to be included in the AI chart conversion, you can click into the note and use your @call or Dot Phrases or free type any additional information.

7. Once you're satisfied with your input, click ChartSmart to begin the AI conversion process.

8. You can choose from 4 types of notes:

  • SOAP Note

  • Procedure Note

  • Surgical Note

  • Follow-Up Visit

You can also create a custom prompt if you have something more specific in mind.

9. We will proactively save your original dictation as an additional note in case you want to re-run the ChartSmart function later with a different prompt. If you do not wish to save your original dictation, un-click this box.

10. Once you've made your selection or created your custom prompt, click Save.

11. Your converted dictation will appear as a new, AI- powered chart note. While still in editing mode, you will see some parts (ie. the headline) in markdown. These additional hashtags will go away once the note is saved.

To see the final note format, you can click Preview.

12. In Preview mode, you can see the the final version. To continue editing, click the Edit button.

13. While you are still in Editing mode, you can add information to your chart with the @call or Dot Phrase functions or by typing your additional content.

14. Once you're finished, choose how you wish to save your chart, and complete it.

Viewing Full Encounter Notes

15. Once you've completed your ChartSmart function, you will see a preview of the full chart in the Encounter Note area on the Patient's timeline. To view the entire note without clicking into the procedure, click View Entire Note.

16. A modal will pop up that includes a scrollable version of the entire note. This is a view-only field. To edit the note, you will want to re-enter the chart. Once you have completed reviewing the note, click the X at the top to exit.

Viewing Saved Dictations & Adding Additional Procedure Notes

17. Whether you prefer to review or use your original dictation or add a new procedure note, click View Notes from the Timeline on either a Health Procedure or Cosmetic Procedure.

18. If you selected to save your original dictation, you will see it as a procedure note.

19. If you would like to re-run your ChartSmart function using a different format or your own custom prompt, click Edit.

*Please note, re-running the ChartSmart function will count as 1 chart.

20. Follow the same ChartSmart functions as you did with the initial note.

21. For instance, where we originally asked for a Follow-Up Visit, this time we can use the SOAP Note prompt.

22. Click Save.

23. You will now see the note has been reformatted in a SOAP Note structure

24. Since this note was created in the Procedure Notes section versus the Encounter Note section, if you want to swap your main Encounter Note, you can click Copy and paste it into the Encounter Note area of your main chart.

25. You can also add a new note with a new dictation by following the prior steps. Click Start Dictation & grant your browser permission if you have not done so.

26. Dictation works the same way in the Procedure Notes section. While you are dictating, the transcript will not appear but will once you've clicked Stop Dictation, and it's had a few seconds to work. Once finished, you can complete the same ChartSmart steps to create your new note.
