The Medical History Form can be accessed in the Patient Portal (Patient Facing) and from the Aesthetic Record App or Website (Clinic Facing). This information lives in the clients profile and can be updated at any time. You can also use the @call function while note taking to bring any information from the form to your notes.
Medical History on the Web
1. Log in to AR on the web.
If the clinic wants to fill in, edit, or view the patients Medical History, simply click the Patients/Clients tab at the top.
2. Once you have clicked into the desired patients profile, click "Medical History".
3. From here, the clinic can fill in, edit, or view the patients medical history.
Alert: If the form has the Allergy Section filled in, it will badge the client profile with a red caution sign to alert you of any potential risks.

4. Log in to Aesthetic Record from the app. Tap the bubble icon in the lower right hand corner, then the magnifying glass icon. Type in the patient's name in the search box and tap the profile.
5. Tap the three dots in the top right corner. Press Medical History.

6. This information can then be pulled with the @call function during the note taking of a procedure. Simply type @call and select the information you want to auto-populate into your notes.

7. The patient can view/edit their medical history through the Patient Portal as well. After logging in to the portal, they can click on my Medical History and update from there.
